Your Responsibilities:
As a Home Health Occupational Therapist, you will:
Partner with the Case Manager to evaluate and address patients' needs within their home environment.
Provide skilled therapy services focusing on home safety, medication management, pain management, fall prevention, wheelchair seating/positioning, and chronic disease management.
Offer family caregiver training to support patient well-being.
Serve as an educator, patient advocate, and integral member of an interdisciplinary care team.
What We're Looking For:
Licensing & Certifications:
Current, unrestricted Occupational Therapist license in California.
Current BLS/CPR certification.
Valid CA Driver's License.
Skills & Experience:
Home health care experience preferred.
Experience working with high-acuity patients is a plus.
Strong knowledge of local community resources.
Familiarity with home health regulatory guidelines (preferred but not required).
Case management fundamentals and interdisciplinary team collaboration experience highly desired.
Exceptional interpersonal, leadership, and organizational skills.
Self-directed, empathetic, and a critical thinker.

Compensation:
Salary Range: $120,000 - $130,000 per year (commensurate with experience).
Generous bonus incentive plans.
